Rocket Card Replacement Scholarship
Rockets who have designated their name differently than their legal name via the myUT
portal are able to have their name appear on the front of their Rocket ID card with
the legal name appearing on the back of the card. Replacement Rocket ID cards have
a cost associated with them; if you would benefit from a scholarship to cover the
cost of the replacement card, please complete the below application.
